  • Last tax return and W-2 form (2 years if self-employed)
  • Copies of most recent investment statements
  • 2 months of your most recent checking account statements
  • A copy of current Homeowners Insurance Policy (if applicable)
  • Divorce Decree (if applicable)
  • Purchase contact/Sales contract
  • Social Security of Pension Award letter (if applicable)


  • Current and former employers’ contact information (names, addresses, and phone numbers) 2 year’s history
  • Housing expenses (monthly rent or mortgage payment amounts, insurance payments, real estate taxes, homeowners, and association dues, etc.) and Landlord name and contact information
  • Liquid Assets (i.e., assets that are easily convertible to cash), including checking, saving and money market accounts, trust funds, etc.
  • Other liabilities, including accounts and balances that might not be reported on your credit report
